Survey Incentive Terms and Conditions

This document outlines the general terms and conditions for participation in survey incentives offered by Baxter Research Center.

These terms apply ONLY to survey invitations that explicitly offer a participant incentive and include a direct link to this page. If your survey invitation did not include an offer of an incentive, these terms do not apply.

1. Offer Details

The specific reward, offer period, and any additional details are defined in the survey invitation email you received.

2. Eligibility

Incentive offers are open to legal residents of the United States who are 18 years of age or older and who have received a direct, personal invitation from Baxter Research Center to participate in a specific, sponsored survey. All offers are non-transferable and limited to one (1) redemption per person per survey.

3. How to Qualify for the Reward

To qualify for and receive the stated reward, a participant must complete the following steps before the offer end date:

  • Use the unique survey link provided in the official email invitation.
  • Answer all qualifying questions asked at the beginning of the survey in a manner that meets the eligibility criteria to proceed and qualify for the reward.
  • Complete the survey in its entirety. A "qualified submission" requires a good-faith effort to provide thoughtful and coherent answers to a substantial number of the questions.
  • Provide your name and a valid email address on the secure claim page presented after survey completion.

Baxter Research Center reserves the right to disqualify any submission that is incomplete, appears to be automatically generated, or does not reflect a genuine attempt to provide feedback.

4. Reward Fulfillment

Once your submission is verified as complete and valid, the reward will be delivered as described in the survey invitation (e.g., electronically). Please allow the processing time stated in the invitation (typically 7-10 business days) for delivery. Baxter Research Center is not responsible for non-delivery of a reward due to an invalid or incorrect email address being provided by the participant.

5. General Conditions

  • Rewards are not transferable and may not be substituted for cash. This offer is void where prohibited by law. Baxter Research Center reserves the right to modify or cancel any incentive offer at any time.
  • Baxter Research Center is not responsible for technical malfunctions, fraudulent activities, or system errors. Any attempt to undermine the legitimate operation of this incentive program may be a violation of civil and criminal laws. In the event of an error resulting in the accidental award of an incorrect prize, our sole liability will be limited to the intended incentive value for qualified participants.
